You will need either:
- A current Passport (less than two years expired) and current Drivers licence/Proof of age card; OR
- A, full birth certificate/citizenship certificate, current Drivers licence/Proof of age card and a Medicare card
- If you have changed your name since birth, you will also require a Marriage Certificate or Change of Name Certificate from Births Deaths and Marriages
